Netcare 911 paramedics responded to a call involving 3 trucks. Four men were occupants in a truck that rear ended another one and pushed it off the road. Two occupants in the first truck were killed during the collision, they were thrown out of the vehicle and landed in the centre island as the truck went across the centre divide directly into on coming traffic.Motorists took evasive action as the truck continued for about 300 m on the wrong side of the freeway before coming to a stop.

The two other occupants were treated and stabilised on scene before being transported to hospital by ambulance. The third truck came to a standstill in the centre of the north bound side with only slight damage.

Metro police took control of the scene as traffic was affected in both directions.

Fatal truck accident and accident at same location, Rivonia

Fatal truck accident and accident at same location, Rivonia
Report on a truck accident occurring on the N1 South Rivonia, 16h00. The truck involved was carrying approximately 20 tons of cement (in bags) when it crashed through the armco barrier and down the embankment taking a car with it in the process.

The driver of the truck suffered a severe, fatal head injury as the heavy load shifted forward and sheared the whole cab from the chassis. One passenger from the truck was injured and transported to hospital. A total of 8 vehicles were involved, only one was able to be driven away, the rest were all towed from the scene with a number of people having sustained less severe injuries who needed medical attention.

• Second incident, same location

By 23h00 last night the scene had not yet been cleared. Two heavy duty recovery rigs capable of lifting approximately 45 tons and 80 tons respectively were required on scene.(and were on scene.). The freeway had been totally closed between Rivonia and William Nicol to ensure a safe recovery.(Direction South)

During the recovery operation a car came down the freeway on the Southbound side but heading North.(on the wrong side of the road) It was reported that the driver got onto the freeway at William Nicol, on the wrong side and proceeded all the way to Rivonia, just over 4km, to where the first incident was still in progress.

The driver of the light motor vehicle then crashed into a tow truck which was still on scene at the first incident and was assisting with the road closure. The female driver of the car was seriously injured in the collision. She was treated on scene by Netcare 911 paramedics and transported to Netcare Sunninghill hospital.

She had sustained leg and arm fractures as well as injuries to her head and chest.
She is currently in ICU.(Photos of the car and truck taken in daylight at a different location to show impact damage). The scene was finally cleared at around 03h00 this morning.

Fatal truck accident on N12 after driver takes evasive action

Fatal truck accident on N12 after driver takes evasive action
A fatal truck accident occurred earlier today on the N12 and underneath the R24 bridge. One truck was involved and reports from the scene indicate the driver took evasive action as the driver of the car in front of him experienced a tyre blowout.

The driver was declared dead on scene once access could be gained with assistance from rescue personnel. No other injuries reported. Traffic was held up on both sides of the N12 for several kilometres.

Traveling towards Johannesburg was impossible for some time as all lanes were closed, then only one was reopened, as the recovery of the truck was not yet complete.

Driver Error in Fog Kills Two in 100 Car Pileup in California

Driver Error
Driver Error

18 Tractor Trailers were involved in a 100 car pileup in a California fog on November 4, 2007, and the tragedy is the Federal Motor Carrier Safety Regulations require drivers to use "extreme caution" if they have poor visibility. This was an avoidable collision for the driver's, and the Innocent's hurt by their recklessness, if the trucktractor trailer drivers had only followed the law.

Specifically 49 C.F.R. § 392.14 states a driver must exercise "extreme caution" when hazardous conditions, such as those caused by snow, ice, sleet, fog, mist, rain, dust or smoke, adversely affect visibility or traction. Had the driver's exercised "extreme caution" there is no doubt in my mind that there would have not been 18 tractor trailers involved in this collision and the photographs I am pasting below would not have included a tractor trailer
